Mycocide Nail Fungus Test Kit Instructions

Woodward Laboratories now offers you the opportunity to have an initial screening performed without the time, expense and embarrassment of a visit to the doctor. The Mycocide Nail Fungus Test Kit gives you all the tools you need to collect a sample of the problem nail in the privacy of your home. By mailing the sample to our lab, you can have the test performed and results provided with complete confidentiality. Step 1: Read all instructions and complete all sections... >
Read all instructions and complete all sections of the Personal Information Form. When completed, place the Personal Information Form inside Postage Paid Return Envelope provided.
Step 2: Complete the information on the Sample Identification Labels... >
Complete the information on the Sample Identification Labels. Peel one label off from the backing and apply to the Specimen Collection Pouch. Place the remaining label in the postage paid return envelope.
Step 3: Gather the Nail Clipper and Wooden Stick from kit.. .>
Gather the Nail Clipper and Wooden Stick from kit, and place a paper towel under the foot or hand with the problem nail. Open Specimen Collection Pouch with label affixed and keep nearby.
Use the Wooden Stick to collect as much debris as possible from the nail indicated on the Personal Information Form. Do not dig under nail or try to lift up nail edge.
Use the Nail Clipper provided* to collect the nail sample from the same nail used to collect the debris sample. Clip the nail as close to the skin as possible without cutting the skin or causing pain.
*Do not use your own nail clipper to avoid cross-contamination.
Step 4: Place the nail clipping and debris sample into... >
Place the nail clipping and debris sample into the Collection Pouch and seal closed.

It is important to obtain debris from under the nail as well as a nail clipping from as close to the skin as possible without cutting the skin or causing pain.
Step 5: Be sure specimen collection pouch is secure... >
Be sure specimen collection pouch is secure and place inside the Postage Paid Return Envelope provided along with other label.
Seal envelope and mail to Woodward Laboratories.

Test results are private and confidential. Within 2 weeks from the date that the specimen is received at the laboratory, results will be made available to you by email, mail or fax.
